Field of Dreams Membership Contract Agreement
Membership is available for FREE for anyone with a special need. Once submitting this agreement, please visit the complex during our staffed hours (9am-dusk, Monday-Saturday) WITH THE FOLLOWING:
- Special needs loved one
- IEP/Dr. Note/any documentation showing diagnosis (we do NOT need to keep a copy; can be shown digitally or physically)
A contract agreement must be filled out for each special needs person.
Rules and Code of Conduct
In order to ensure this complex will be maintained and utilized for years to come, the TRFOD asks for you to comply with the following:
- Members will be able to utilize the complex from 8am-dusk, Monday through Saturday, from April to November.
- The TRFOD holds the right to close the complex during these times for any of the following reasons: weather, construction/repair, private events, and organized partnerships with school districts and special needs organizations. Please visit our website (rwjbhfieldofdreams.com) for our weekly schedule.
- The complex is equipped with video surveillance and facial recognition software. The use of the complex by non-members is strictly prohibited.
- The TRFOD holds the right to revoke any private membership for the following: vandalism, inappropriate behavior, entering outside of hours, allowing more than the designated amount of people in your family to enter, behavior that is not appropriate or kind to other persons.
- The TRFOD will have times during which staff and volunteers are available to assist your family. During these times, items for play will be available. If there are no TRFOD personnel present, these items will not be available.
- You are responsible for your child/loved one at all times while in the park. Our staff/volunteers are not responsible for supervising your loved one while in the park.
- Use the complex and equipment at your own risk. The TRFOD cannot be held responsible for any injury.
Your digital signature below confirms that you and your family understand the rules and regulations of the complex. Your signature also confirms that you consent for your picture to be taken and used on our social media and website.
Authorized Vizpin Users
Our complex remains locked at all times, to ensure the safety and security of our complex and our members. Our organization uses the Vizpin digital key system to allow registered members to enter the complex freely. Each family can have up to 2 Vizpin users (over the age of 18). The selected users should be adults who will most frequently be accompanying your special needs loved one to the park.
Our staff/volunteers will further explain the Vizpin process upon your first visit to the park with your documentation.
