This will be the driving force behind tracking all online giving so be sure to customize it with all the pertinent details. You can use this page to let everyone know they can donate safely and securely. Please note, all donations made through your Children's Champ fundraising page are directed to Children's Specialized Hospital.

*I agree that if the event I wish to hold is approved by Children’s Specialized Hospital, I agree to abide by the Community Fundraising Event Guidelines, as detailed in this application. THE EVENT ORGANIZER AGREES TO ASSUME ALL RISKS AND LIABILITIES ASSOCIATED WITH THE EVENT AND HEREBY RELEASES AND HOLDS HARMLESS CHILDREN’S SPECIALIZED HOSPITAL AND ITS DIRECTORS, OFFICERS, EMPLOYEES, AGENTS, SUCCESSORS, AND ASSIGNS FROM AND AGAINST ANY AND ALL CLAIMS, DAMAGES, LIABILITIES, COSTS AND EXPENSES, INCLUDING REASONABLE ATTORNEY’S FEES ARISING OUT OF OR WHICH MAY OCCUR IN CONCURRENCE WITH THE EVENT, INCLUDING, WITHOUT LIMITATION, ANY PERSONAL INJURIES OR DAMAGE TO PROPERTY WHICH MAY OCCUR IN ASSOCIATION WITH THE EVENT. It is also agreed that funds raised from the event will be paid to Children’s Specialized Hospital within 30 days of the event or within alternate terms mutually agreed upon.
